Soothing a Sore Throat: Mānuka Honey vs. Over-the-Counter Syrups

When a sore throat hits, most people reach for an over-the-counter syrup. But there's a natural alternative that's been used for centuries: Mānuka honey. How do they really compare?

What's Inside?

Over-the-counter cough syrups typically contain active ingredients like dextromethorphan (to suppress coughing) or guaifenesin (to thin mucus). They also come with a long list of inactive ingredients: high fructose corn syrup, artificial colours, preservatives, and sometimes even alcohol.

Mānuka honey contains one thing: honey. That's it. No synthetic additives, no preservatives, no mystery ingredients.

Which Works Better?

Research suggests honey holds its own — and sometimes wins. A major 2018 review from Oxford University analysed 14 clinical trials and found that honey was superior to usual care for relieving upper respiratory tract infection symptoms, including sore throat and cough frequency.

Mānuka honey has an extra edge: its MGO (methylglyoxal) content provides natural antimicrobial and anti-inflammatory properties that regular honey doesn't have. Laboratory studies have shown it can even inhibit Streptococcus pyogenes — the bacteria behind strep throat.

OTC syrups work for specific symptoms. Dextromethorphan reduces the cough reflex, but does little for throat pain. Guaifenesin thins mucus but doesn't soothe inflammation. They're effective for their intended purposes, but they don't offer the broad, soothing relief that honey does.

The Side Effect Difference

This is where the two diverge significantly.

OTC syrups commonly cause:

  • Drowsiness (dextromethorphan)

  • Nausea and dizziness

  • Digestive upset

Mānuka honey's side effects are nearly nonexistent for most people. The only real concerns are rare — minor digestive discomfort if taken in very large amounts, or an allergic reaction for those with bee product allergies.

The World Health Organization and the American Academy of Pediatrics both recommend honey as a first-line remedy for cough in children over one year — precisely because it works without the side effects.

How to Use Mānuka Honey for a Sore Throat

  • Take 1 teaspoon of Mānuka honey (UMF™ 10+ or higher works well)

  • Let it sit in your mouth for 10–15 seconds before swallowing

  • Repeat every 2–3 hours as needed

  • For extra soothing, mix with warm water, lemon, and a little fresh ginger
